Privacy Policy
This MedFN Privacy Policy (“Privacy Policy”) states MedFN’s commitment to maintain each
member’s right to the privacy, confidentiality and security of their personal information and
any other information they may provide to MedFN through our website, or any other communications with MedFN.
Personal Information
“Personal Information” means any information that may
be used to identify an individual, including, but not
limited to, a first and last name, home or other physical
address, an email address, phone number or other contact
information, whether at work or at home. MedFN does
not collect your Personal Information unless you voluntarily
provide it to us on our website at www.MedFN.com (“Site”),
or any direct communications with us, or by registering
to become a member of MedFN.
In terms of health care providers, your doctor (and his or her staff) will only be able to access or
review your Personal Information, including your medical records, if you give such doctor a unique MedFN
number for information identified as “Public” information, or an additional PIN for information identified
as “Privileged“. If you choose to give a physician access to your MedFN account, it is your responsibility
to monitor and control such physician’s access.
MedFN is not responsible for monitoring access to your
account. You acknowledge and agree that MedFN shall
not be liable for any information added to your MedFN
records by you, or other party granted access, nor shall
MedFN be responsible for any disclosures made by you,
or other party granted access, regarding the information
in your account.
Anonymous Information
We may collect certain anonymous or non-identifiable information about you when you visit our Site
or when you answer our questionnaires or surveys. We may share such anonymous or aggregate information
with trusted third parties or use such information to shape and direct the creation and maintenance of
content on this Site or to improve the service levels of the Site.
Access to
Records
When you subscribe to our services, you will be provided
with a MedFN number and will be asked to choose a PIN,
User Name and Password (together these are your “Access
Codes”). These Access Codes will be used to identify
you when communicating with MedFN or its agents and
to allow online access to your records via our Site.
You agree that it is your responsibility to safeguard
the confidentiality of your Access Codes both online
and offline. You will need your Access Code to view
your records and to make any changes to any Personal
Information, including, but not limited to, medical
records or other information that you may have provided
to MedFN.
You acknowledge and agree that MedFN is not responsible
for any adverse condition or event, including any violation
of your rights of privacy or misuse or changes to your
medical information that may be caused, in whole or
in part, by your failure to safeguard the confidentiality
of your access codes. You acknowledge and agree that
MedFN is not a provider of medical or health care services
and does not practice medicine, or give medical advice.
You understand and agree that MedFN does not edit the
contents of your chart or render any opinion or judgment
regarding your medical health or physicians’ care.
Membership
MedFN services are designed and intended for use by
adults, and are not intended for, nor designed for use
by children under the age of 18. Parents or guardians
of children under the age of 18 may register them for
our service only under the condition that the parent
or guardian manages the personal healthcare information
for such child directly. The parent or guardian is responsible
for providing supervision to ensure the minor's authentication
information is kept secure and the credibility of the
health record is maintained.
Emergency Contacts
MedFN allows you to identify family members or friends
who will serve as emergency contacts for you in the
case of an emergency. After registration for the MedFN
services, you may provide the name, address, phone number
and relationship to you for each of your emergency contacts.
It is your responsibility to contact the individuals
you wish to provide as emergency contacts in order to
request the right to designate them as an emergency
contact and to provide their personal information to
MedFN for such purpose. Only the individual that is
listed on the MedFN account as the member or if the
member is under the age of 18, the parent or guardian
on the account, can change the individuals listed as
emergency contacts, update the information for emergency
contacts and remove or add emergency contacts.
MedFN will not contact your emergency contacts to inform them that they are emergency contacts.
It is your responsibility to contact individuals you want to serve as your emergency contacts and to
receive their direct permission to list them as an emergency contact in your MedFN medical record and to
provide their contact information to MedFN.
Information
Provided to Third Parties
MedFN does not rent, sell or share Personal Information it collects about you to or with third parties.
Personal Information collected from you is only used to complete and support your purchase of Services from
and use of the Site. MedFN may disclose Personal Information if required to do so by law or in the good faith
belief that such action is necessary to: (1) to comply with legal process served on MedFN or this Site; (2)
protect and defend the rights or property of MedFN; or (3) act in urgent circumstances to protect the personal
safety or life of users of MedFN, its web sites or the public. If MedFN should ever file for bankruptcy or
merge with another company, we may transfer the information you provide to us on this Site to our successor
entity if such entity intends to continue to provide services similar to those provided by MedFN.
Cookies
MedFN may use a session cookie in its website for performance and reliability. A cookie is a small file
stored on the site user's computer or Web server and is used to aid Web page navigation. A session cookie
is a temporary file created whenever a website is accessed and is self-terminated based either on an expiration
date (e.g. 3 hours from creation of the cookie) or by closing the Web browser. A persistent cookie is a permanent
file and must be deleted manually. MedFN does not use persistent cookies.
Third Parties
MedFN may contract with third party vendors to provide services and information to our members.
MedFN uses best efforts to ensure all such vendors comply with the privacy and confidentiality policy of MedFN.
This Site may contain links to other sites for informational purposes. MedFN is not responsible for the privacy
practices or the content of any web sites linked from this Site, nor does linking to other web sites indicate any
endorsement of such sites or the products or information provided on such sites.
Security
MedFN has security measures in place to protect the
loss, misuse, and alteration of the information stored
on the system. This Site is part of a private network
that does not provide direct access by outside parties
to the servers. Firewalls are in place to prevent unauthorized
access to equipment and servers. All systems are continuously
monitored for suspicious activity to prevent any breaches
in security.
